Questions on forms are powered by Activity Types. Each question must be stored in an extra field that Donorfy can display and capture on your form.
Create an Activity Type for Your Questions
Navigate to Settings,click Configuration.
Select Activity Types.
Click the plus icon to add a new Activity Type.
Enter a Description for the Activity Type (e.g., Health and Safety).
In the Extra Fields section, enter your questions,Yes/No fields are ideal for questions such as:
“Read the Safety Guidelines?”
“Agree to the Terms and Conditions?”
“Ready to have an awesome time?”
Arrange the fields in the order you want them to appear.
Click Save.
📌 Note: Each question must be stored in a separate Yes/No field.
Add the Questions to Your Donorfy Form
Open your Donorfy Form.
Add an Activity Element to the form.
Choose Add an Activity without asking a question.
Select the Activity Type you created earlier.
Under Use the extra fields as follows, choose:
Answer question with a Yes/No answer
For each Yes/No field, choose whether it should appear on your form.
Enter the full question text in the text box provided.
Save your form.
