To add a question to your form you will need to start with an Activity Type where you will record the questions, for example, Health and Safety. This will need to be set up from within Settings > Configuration > Activity Types
Click on the + icon to add a new Activity Type.
Next, enter the description.
In the Extra Field's boxes is where you will ask your questions. For Yes/No type questions such as:
Read the Safety Guidelines?
Agree to the Terms and Conditions?
Ready to have an awesome time!?
These would be added to the Yes/No boxes:
Once you have all your questions in and in the correct order, you can save the Activity.
In your Donorfy Form is where you can then use these as part of a question.
Add an Activity Element to your Form and then choose to add an Activity without asking a question.
Set the Activity Type as the one you created before
Then under the section "Use the extra fields as follows"
Select from the drop-down boxes "answer question with a Yes / No answer"
Then for each of the Yes / No fields that you created you can opt to use the field or not.
Write your full question in the box below, e.g:
When you have finished, save the changes.
Here is an example of how they will then appear when a volunteer is competing the Form - I have highlighted them for you:
